This form is to be completed by a parent or legal guardian for all students of the Children's and Youth Ministries of Suburban Christian Church. Responses will remain confidential and will be released only as necessary to individuals within our ministry leadership in order to ensure the safety of the students and confidentiality of the information provided.


Additional contact in the case of an emergency:
Occasionally, Suburban Christian Church would like to use photos or videos of children in church literature, displays, and website. Children will not be identified by name in any literature or on the website.

It is the policy of Suburban Christian Church that any child from birth through 5th grade will be released from any ministry activity only into the custody of an adult parent or legal guardian as listed on this registration form. Authorization for release of a child to anyone else must be provided in writing. This portion of the registration form is to be used to designate additional person(s) that your child can be released to. Anyone named must be 18 years of age or older.
When picking up your child, the designated person MUST present the appropriate security document (example: pager and/or parent/guardian card). Additionally, the person may be asked to provide proof of identification.



This portion of the form should be completed for children ranging from birth through the 5th grade.
As the parent or legal guardian of the child named above, I authorize Children's Ministry workers at Suburban Christian Church to release this child from Sunday School, Children's Church, Nursery, and AWANA to the following person(s):

  • I understand that anyone attempting to pick up my child must presnt the appopriate security document before the child can be released, and that it is my responsibility to ensure that this document is provided to that person. I will also make sure that the person knows that they may be asked to provide proof of identity in addition to the security document.
  • I understand that this authorization is only valid for the ministries specified on this form.
  • I understand that unless specified, this authorization is valid until Septempber 2027
  • I understand that I may revoke this permission at any time by making a written request.
